cyberpunkdreams is a massive (over 2.8 million words) cyberpunk interaction fiction/RPG. It's free to play on Steam. It's written on a custom engine and also features over 3,000 unique hand-drawn images. All of this is currently growing. The game is in full release and has been for a few years, but gets regular updates.
Although it's not primarily an adult game, it contains a lot of adult content. Romanaceable NPCs, one-off encounters, interactive sex scenes, third party sex scenes via "spikes" that you plug into your brain, and more. And, of course, there are transformation themes. Mostly these are under the player's control and take the form of surgery, cybernetics and mutation, but there's an element of mind control and horror in some areas too (those "spikes" can also be used for other things...) Players can also start with sexual mutations, such as extra breasts and testicles or a literal horse penis. There are also a lot of wardrobe options. On the mental side, the player character can evolve their mindset and ideas over time, including picking up new fetishes and sexual quirks and altering their gender.

Each gender stat moves on a sliding scale, and if you chose to be non-binary, there are a variety of sub-choices you can make (agender, demigender, etc). You can also choose to be consciously trans, including post-op, and what this means to you. All of this will have an impact on your experience in the game.
All in all, there are a lot of combinations, and more planned (the items ticked on the list are only what's in the game now). Overall, it takes a semi-realistic approach to transformation -- surgery and pharmaceuticals, not magic wands.

Lots of tweaks and new stuff, as ever, but pertinent to here:
While I enjoy the fresh take on the RPG genre with the cards, it is one of the least user friendly games I have played, and the tutorial hand holding is not enough. I'd even suggest that the little tutorial there is is almost doing more damage than good. Immediately it starts asking you to use stuff from your inventory, but you won't have access to it for a long long time, leading to much confusion searching through the UI.
The conditions, knowledge, rumors and stuff and how to manage them are equally confusing, and made even more so by being half trapped behind progression. The lack of save files combined with percentage rolls for save or suck/die rolls makes it hard to invest in your character.
The 50 actions per day limit, seems like a lot when you are just starting out and every action is an eternity of confusion, but as you start to move faster it immediately becomes a source of frustration when all you did today was earn a handful of bottle caps and earn some rumors that were all made useless by a few bad rolls, and you don't really had an adventure worthy of a memory.
While this review might sound all negative, I still think there is gold hidden in this game, but the frustration points are overwhelming, and to get to a point where the frustration turns to love requires a lot of investment in it's complex machinery.
I lived through the early MUD days, and maybe that's where I can find some rose colored glasses for this game. It's horrid and unforgivable, but for those willing to run the gauntlet, there could be memories that last a lifetime.
If you like slow-burning RPG's (with an emphasis on the 'roleplaying' aspect), it's hard to do better than Cyberpunkdreams. Original and fleshed-out cyberpunk setting, incredible depth in terms of both writing and player choice, and a really fleshed out economic system make this an easy recommendation. The action point/energy system takes a while to get used to, but if you accept that this is a game intended to be played over a period of months, not hours, it really adds to a sense of pacing (plus it's nowhere near as restricting as it first appears). CBD is a challenging but rewarding experience, and once you start to understand how it's (many) interwoven systems work, it is incredibly satisfying to play.
There are a handful of transformation systems to speak of. The first and most obvious is through cybernetics, a staple of the cyberpunk genre. From mental implants to robotic limbs to mechanical eyes to literal core replacements, it's all there. The second is in the form of mutations, which can range from being a vampire to having internal armour to having a literal horse dick. VR-like spikes can also cause TF-like changes. Breast enhancement/reduction surgery are there too, and full genital replacement surgery is planned for later this year. Gender is completely fluid in-game so it all fits from a thematic standpoint, and the clothing system accounts for it from a gamplay perspective. There are also slow mental transformations that can affect the PC, and many of the bigger changes (such as say, breast augmentation) can take time for the PC to fully accept.
If an incredibly deep but slow-paced role-playing game set in a cyberpunk world interests you, I fully suggest giving it a try. That being said, while there is a lot of (optional) adult content and it is absolutely an adult game, it's by no means a porn game. If you're looking for a shorter experience or are mostly on this site for smut games, this might not be for you.
